Growth measurement and Procedures , Pediatric Care

The accurate assessment of growth is important throughout childhood :
* To assure the health of child
* To identify the impact of disease.

Growth Measurements :

– Length

– Height

– Weight

– Head Circumference

– Chest Circumference

1- Length :
Until a child is  2 years of age the length = supine position

Preparation :
Have the parent remove any hat or shoes the infant is wearing

Equipment and supplies :

Measuring board or other length- measuring device

Procedure :

1. When using a measuring board, place the infant’s head against the top of the board .

2. Use the parent or an assistant to hold the infant’s head in the midline and gently push down on the knees until the legs are straight .

3. Position the heels of the feet on the footboard and record the length to the nearest .

4. Repeat the measurement for accuracy . If a differences between two readings is found, take the average reading for documentation .

5. Plot the measurement for the child’s age on the standardized growth curve .

If such a measuring device is not available:

1.place the infant on a paper sheet. Make one mark at the vertex of the head and another at the heel .

2.Then measure the distance between the two marks . Record the length in centimeters or inches

2- Height :

After 2 – 3 years standing upright a gainst a wall with a stadiometer or platform scale .

Preparation :

Have the child remove the shoes

Equipment :

Stadiomter or platform scale with stature-measuring device .

Procedure  for measuring height

1.With shoes removed , have the child stand straight with the back to the wall. The head should be held erect and in the midline position .

2.The shoulder, buttock , and heels should touch the wall.

3.Move the head piece down  to touch the crown .

4.Make the height reading to  the nearest 0.5 cm .

5. Plot the measurement for the child’s age on the standardized growth curve .

In older child and adolescent height is often measured using a platform scale with an attached stature-measuring device.

1. Move the stature-measuring device  to the top of the head .
2. Have the child step off the scale and read the height in centimeters or inches.

Weight :

– Infant is weighted on infant scale with removal of cloth and diaper.

– Either in a supine or sitting position, depending on the age.

Preparation :

1.Have the parent or assistant remove all the infant’s clothing and diaper .

– Weigh toddler in their underclothes .

– Weigh older children in their street clothes

– with heavy clothing and shoes removed .

2. Clean the infant scale tray between uses. Place cover over the scale tray .

3. Check the balance of the scale before using it

Equipment :

Stadiomter or platform scale with stature-measuring device .

Weight measuring Procedure for infant:

1.Place the infant on the scale and keep a hand close .

2.Distract infant, and take the reading when the infant stops moving .

3.Record the weight in the nearest 10 g .

4.Plot the measurerment for the child’s age on the standardized growth curve .

Weight measuring Procedure for older children:
On standing scale

1. Have the child stand still on the scale .

2. Move the weights until the scale is balanced for the child’s weight .

3. Record the weight to the nearest 0.1 kg .

Head Circumference :

Is usually measured at regular interval until the child’s second birthday .

Preparation :

Remove any hat the infant wearing

Equipment :

Disposable, nonstreching measuring tape with centimeter and millimeter marking.

Procedure :

1. Wrap the tape around the head at the supraorbital prominance , above the ea rs, and around the occipital prominance .

2. Record the circumference in the nearest 0.5 cm. Repeat the measurement to confirm the reading .

3. Plot the measurement for the child’s age on the standardized growth curve .

Chest Circumference :
Measured until 1 year Is useful measurement in comparison with head circumference when there is concern with the growth of either the head or chest .

Equipments :

Disposable, nonstreching measuring tape with centimeter and millimeter markings .

Procedure :

1. Wrap the tape around the chest ,placed just under the axilla and at the nipple line .

2. Record the circumference in the nearest 0.5 cm. Repeat the measurement to confirm the reading .

3. Compare the chest circumference with the head circumference measurement.

The head circumference = chest circumference until after 1 year of age

Abdominal Circumference ( Girth ) :

May occasionally be measured in children to monitor abdominal size that can vary such as edema from cardiac , renal diseases .

Equipments :

Disposable, nonstreching measuring tape with centimeter and millimeter markings.

Procedure :

1. Wrap the tape around the abdomen , at the level of the umbilicus, taking care to prevent a paper cut .

2. If the measurement is taken at another location on the abdomen, place ink marks at the location of the measurement .

3. Record the measurement in the nearest 0.5 cm.

4. Compare the reading with those taken previously to determine a change in size .
