–    Used to document assessment in three areas
–    Eyes
–    Verbal response
–    Motor response
    Normal is 15 and less than 8 indicates coma.
–    Gives an idea about the level of consciousness in scores.

On comands ……………3 scores
On Pain …………………2 scores
Nil ………………………1 score
Best Motor response
   Obey …………………….6 scores
localize pain……………..5 scores
Flexion withdrawal…….. 4 scores
Flexion abnormal..………3 scores
Extension……..………… 2 scores
Nil ………………..…….. 1 score–    Verbal response
    Oriented ……………………5 scores
Dioriented………………….4 scores
Inappropriate words ………3 scores
Incomprehensive sounds.. 2 scores
Nil………………………….. 1 score
The total is 15 scores , less than 8 mean comatose patient , increased mean improvement , decreased = deterioration
