Cleft Lip and Cleft Palate


Definition of Cleft Lip (CL)

– Is a facial malformation, physical separation of the two sides of the upper lip and appears as a gap in the skin of the upper lip. This separation often extends beyond the base of the nose and includes the bones of the upper jaw and/or upper gum.

Is a split or opening in the roof of the mouth. A cleft palate can involve the hard palate (the bony front portion of the roof of the mouth), and/or the soft palate (the soft back portion of the roof of the mouth)



• The defect appear at birth, unilateral or bilateral

• Nasal distortion

• Deformed or absent teeth

• Difficult in suckling and swallowing

• The deformity involve soft and hard palate

Problems Associated with Cleft Lip and/or Palate

– Eating problems

– Ear infections, hearing loss, recurrent otitis media

– Speech problems

– Dental Problems ( malposition of teeth)

– Upper respiratory infection

Nursing intervention in Cleft Lip and/or Palate

• Preoperative care:

– Provide psychological support for the family

– Modify feeding techniques

– Hold child in upright position

– Use special feeding equipments (large, soft nipples with large holes)

– Bubble frequently

– Explain the immediate and long term problems

• Postoperative care:

– Position on back or side lying

– Maintain lip protective device

– Restrain arms of the child

– Prevent severe crying

– Cleanse suture line gently after feeding

– Teach the parents about:

– Feeding equipments and technique

– Restraining procedure

– Mouth care

– Frequent bubbling
